People in the New England region are dying from cardiovascular disease younger than people from other parts of Australia.
We need to fix this.
This 12 month campaign aims to ensure your patients have the best chance of avoiding death from cardiovascular disease.
You're invited to participate as a foundation member of this new campaign to ensure that your patients have the best chance of avoiding premature illness from cardiovascular disease. Our general practices will collaborate to ensure that we implement what we know is best practice to benefit our patient’s health, while making it easier to provide the excellent care to which we aspire, and improving practice sustainability.

Dr Tony Lembke
A full-time GP in Alstonville, Tony has worked with more than 1000 Australian general practices on strategies to improve their systems for delivering care in his role as Clinical Director of the Australian Primary Care Collaboratives and as director of the Australian Centre for the Medical Home. Tony was the RACGP GP of the Year in 2012 and Sydney University Medical Society Donkey Kong Champion in 1983.
Kim Poyner
Kim has an extensive history working with medical centres and Primary Health Networks to train medical professionals in Behaviour change theory and Chronic disease management. Kim was awarded the 2015 Australian Best Practice Nurse award in Chronic Disease Management and has successfully completed her AMA exams for international endorsement of her coaching qualifications. She is training Health Professionals across Australia in the Health Care home trial, readiness projects, and Quality Improvement strategies.
Katrina Otto
One of Australia’s most experienced Digital Health and medical software trainers, Katrina's focus is on increasing efficiency of health IT and practice systems to benefit both practices and patients. She has worked in a wide variety of medical practices for the past 30 years, mostly as a Practice Manager and works across Australia as a Medical Software Trainer and Practice Management Consultant. Katrina is an approved trainer for: MedicalDirector, Best Practice Software, Australian Digital Health Agency, Department of Health, Avant Mutual Group, AHPRA, RACGP, ACCRM, AAPM, APNA, Pen Computer Systems and others.

The Heart Foundation is seeking doctors, nursing, pharmacy, Aboriginal Health Workers
and allied health professionals who are passionate about improving cardiovascular
outcomes in primary and secondary care to join our Health Professional Ambassador Program
in 2020. Now is your chance to join this exciting program!
Together we can support prevention, earlier detection and better management of cardiovascular
disease. Committed to supporting health professionals in the New England North West Region,
improve patient care and outcomes, the Heart Foundation is offering this professional
development program free of charge.
What’s involved? To complete the Program participants will need to:
• Attend 3 full day workshops in Tamworth during 2020 (dates to be determined).
• Have approval from your manager to participate.
• Have access to email and internet for ongoing communication in between workshops.
• Be an active participant in workshop discussions.
• Commit to the delivery and presentation of a Workplace Quality Improvement Activity project related to cardiovascular care.

TAMWORTH could be seen as ground zero for heart disease in NSW.The Heart Foundation hand-picked Tamworth as the first site in NSW to host its professional ambassador program.Previously, this program has only been launched on a state and territory-wide basis, but it is understood it will be offered exclusively to New England medicos.The scheme, which is being launched in conjunction with the upcoming 2000 hearts symposium, will train local professionals to help manage heart disease within the region.Through the Heart Foundation's ambassador program, the region's chosen doctors will get access to the "latest evidence and advances in practice for patients living with or at risk of heart disease".
The foundation's NSW heart health manager Scott Walsberger said Tamworth had been chosen for the program because of the city's "much higher rates of death from heart disease".
He said the foundation would choose the ambassadors for the region mainly from general practice, Aboriginal health services and potentially physiotherapy.
The ambassadors would be trained to help identify people at risk, manage and reduce heart disease in the region.
Initial plans have the scheme running in Tamworth through 2020, but Mr Walsberger said, depending on the need of the community, it could be extended.
Heart health is front of mind for the region's health corps presently with the Hunter New England Central Coast Primary Health Network (PHN) hosting the first-of-its-kind symposium for local medicos next week.
The "2000 hearts" conference will feature presentations from leaders in general practice, nursing and cardiology.
The PHN has set a lofty outcome from the event: to help delay and prevent at least 2000 cardiovascular disease related deaths in the region.
