Thank you for taking the time to participate in the Rural Communities Project for Glen Innes.

The purpose of this survey is to get your opinions about your health and wellbeing and that of the community of Glen Innes, and surrounding communities in the local area.

The Primary Health Network will use the results of this survey and other information to identify the most pressing problems which can be addressed through community action.

This survey will take about 10 minutes to complete.
