Have Your Say - Community Survey for ACCHO and AMS

Thank you for taking the time to complete the “Have your Say” survey for the Primary Health Network.

The purpose of the survey is to gain the views of our First Nations partners, including the AMS sector and ACCHO organisations, about the health and health service concerns and needs for the Hunter, New England and Central Coast regions.

We will use the results of the survey to improve our understanding of the health issues for our First Nations communities, and help inform our regional Needs Assessment for 2022 to 2025.

Findings from the survey will help to inform the planning and decision making in this process.

The survey will only take about 5 minutes to complete.

The survey will close on Sunday 29th September.

Please note that we do not collect any contact details from those completing the survey, and will not identify any individual responses.

We only report on survey results at a Local Government level.

If you have any questions or feedback about the survey please contact Aimee Berry, Senior Health Planner via email at aberry@thephn.com.au
