The Primary Health Network has commissioned services for the Community Mental Health Continuity of Support (CoS) program.

This program provides ongoing support for clients who previously accessed services under Partners in Recover (PIR), Day to Day Living (D2DL) and Personal Helpers and Mentors (PHaMs) programs, and are ineligible to receive services under the National Disability Insurance Scheme (NDIS).

The Continuity of Support CoS program provides these clients with supports commissioned through the PHN to achieve similar outcomes.

It provides ongoing funding to ensure CoS clients have access to long-term, responsive support.

This page is provided as a forum for CoS Service Providers to share information resources, personal experiences and links that may be beneficial in the delivery of effective and compassionate services.
